We’re excited to announce that Wesley Woods is moving to United Healthcare for the 2026 plan year.

Your 2026 benefits resources are now available for review.   Take a moment to carefully review the resources on this page to help you plan for any coverage changes you may need to make in the coming year.

As you review the information, please pay special attention to the 2026 Benefits Guide, specifically page 9, which outlines important details and instructions regarding transition of care. If you anticipate needing transition-of-care support, we encourage you to alert your current providers now about the upcoming change.

When the time comes to submit transition-of-care forms, the correct form will depend on the medical plan you elect for 2026 (UHC or Surest).  Please review the Transition of Care Forms for UHC and Surest.

Open Enrollment will run Monday, December 1 through Monday, December 15. Be sure to review the schedule for in-person meetings where you can learn more about these updates and take the next steps to secure your elections for the new plan year.
